Use the forms below to report your recent trials, settlements or arbitrations. Reporting your cases is a great way to get your work published. There are three ways to submit your case:
- Fill in all of the information pertaining to your case and click submit. Fields marked with an asterisk (*) are required. A trained specialist will review your report and contact you if they have additional questions.
- Download and complete the Word version of the form. Fax your report to (425) 482-0527 or scan your report to [email protected].
